Why does the patient need home care services? are we seeing the patient?
Type of service(s) needed (Skilled Nursing/PT/OT/ST)
Ordering physician and or Primary Care Physician name, phone and date of last clinic or hospital visit

Documentation to be provided in the referral:
A demographic or Face sheet that includes: patient name, current address and phone number, DOB, marital status, emergency contact/caregiver name and phone number.
Insurance information including: Medicare and other insurance numbers, primary/secondary insurances
Initial signed physician orders, name phone and NPI number of the ordering MD
Name and phone number of the patient’s PCP
Current medication list and current labs (if available)
History and physical/medical summary and discharge summary statement (if available)

Wound care patients

it is important to know the type of wound/s, and the condition and measurements of the wound/s .
Orders need to specify frequency of dressing changes and cleansing and dressing supplies.
Denson nurses can teach caregivers to perform some dressing changes on long term, twice a day or greater dressing changes,

Patients needing IV infusion

please supply specifics of infusion therapy including:
Drug – Dose – frequency – length of deliver – duration of treatment – Labs
Type of infection and culture results
Will the first dose be given in the home? If so, the MD needs to write orders for an anaphalaxtic
type of access (e.g., central, PICC, peripheral, port-a-cath etc.) and insertion date
If PICC is to be pulled by home care staff we need the length of the catheter
Informed patient/caregiver who is able and willing to participate in their treatment
pharmacy providing drug (Denson’s can find a pharmacy if need be)
Denson case managers will coordinate with the pharmacy to obtain dosing times, pump, labs, supplies, and time of first dose at home.
