Denson Home Health provides a multidisciplinary program to control and manage chronic lung disease patients. Denson’s Chronic Lung Disease Care Pathway is tailored towards health promotion, pulmonary wellness, continuity of care & hospital re-admission prevention within our COPD community.

Progressive Chronic Lung Disease Wellness Pathway – How does it Work?
- Transitional care starts at the acute care facility
- Nurse admit visit scheduled and patient seen within 24 hours of hospital discharge
- Front-loaded clinical visits within the first 30 days to reduce re-hospitalizations
- Patient progress report submitted to physician at 2 weeks, 1 month, and 2 months
- Pulmonary data collection, including spirometry to meet PQRS requirements
- Health coaching of patients to self-monitor for symptoms of early exacerbation
- Tele-monitoring capabilities available post discharge
Chronic Lung Disease Screening: Asthma, Emphysema, Job Exposure, Ex-smoker Interventions:
- Chest physiotherapy
- Flu and pneumonia vaccine promotion
- Right-sided heart failure monitoring
- Oxygen Therapy w/ O2 Sat%
- Smoking cessation teaching ’Spacer’ training for optimal bronchodilator & steroid inhaler use
- Nutritional needs assessment
- Infections & IV antibiotic Therapy
- Incentive spirometry training
- Flutter-valve, nebulizers, peak-flow training
- Aggressive pulmonary toileting
Diagnostic Testing & Data Collection
- Diagnostic Testing & Data Collection
- In-home Lung Function Tests = FEV1 test
- (AAT) Alpha-1 antitrypsin test
- Transfer factor for carbon monoxide screen
- Hypercapnia PaCO2 > 45mmHg testing
- Portable chest X-Ray: results within 4 hours
