Wound Management

Denson’s clinicians are highly trained and experienced in wound management, healing processes, and effective communication with physicians on wound treatment modalities, using evidence-based wound treatment protocols.

Our trained RNs perform a comprehensive evaluation. Then we work with the primary care physician or wound care specialist to create an individualized treatment plan. We address co-morbidities, complications and malnutrition that may hinder the healing process. Denson’s uses a clinical pathway to achieve success, starting with our ability to evaluate the type of wound being treated.

Admission Assessment

Evaluate etiology of wound for proper treatment modality

  • Venous Stasis- Verify circulation prior to multilayer compression therapy, change 3xwk.
  • Arterial Stasis- Verify circulation/ revascularization if needed, topical treatment, NO compression
  • Diabetic Ulcers – Assess HbgA1c, control blood sugars, diet education, offloading, and topical treatment
  • Surgical Wounds- Monitor s/s of infection/ dehiscence. Teach proper body mechanics and precautions
  • Trauma- Monitor s/s of infection, fall precautions, topical treatment
  • Pressure Ulcer- Continue to offload, teach pt./caregiver to change positions, pad bony prominences

Collect Labs as ordered:

  • Pre-Alb
  • Sed Rate
  • CBC
  • CMP
  • Wound CX for suspected infection

Collaborate with MD to initiate an individualized nutritional care plan

  • Zinc 220Mg PO Daily
  • MVI Tab PO Daily
  • Ensure 1-3 cans/day with meals
  • Substitute Glucerna if Diabetic
  • Treatment Modalities

Treatment Modalities

Topical Treatment:

  • Alginates– Moderate to heavy exudate
  • Collagens- Adds proteins
  • Silver dressings– Used for suspected infection. (NOT to be used longer than 14 days)
  • Foams- Moderate to heavy exudates
  • Hydrocolloids– Dry- light exudating wound
  • Ostomies – Proficient in all areas of ostomy care

Advanced Treatment

  • Negative Pressure Therapy (Staff received Level 1 Certification through KCI before seeing patients)
  • Multilayer Compression therapy
