Each day, in their own home, patients use the HRS tablet and devices to transmit symptoms and vital sign data.
This information is automatically and securely transmitted to the clinical team within seconds.
Patients’ data is analyzed and their medical team (physician, case manager) is notified if they require a change in their medical care plan.
Patients also have access to a multitude of useful features to help address the causes of readmissions, including:
